RESUMO Objetivo: O objetivo deste estudo foi revisar os aspectos clínicos e patológicos da catarata congênita secundária às infecções por sífilis, toxoplasmose, rubéola, citomegalovírus e herpes simples. Métodos: Trata-se de uma revisão de literatura, na qual foram incluídos artigos de periódicos indexados às bases de dados PubMed®, Cochrane, Lilacs, Embase e SciELO de 2010 a 2023. Resultados: Foram encontrados 45 artigos, e, após seleção, restaram 9 artigos. Além disso, foram adicionados artigos para enriquecer a discussão. A infecção por sífilis está relacionada a alterações corneanas. O citomegalovírus e a toxoplasmose estão relacionados com a coriorretinite e/ou microftalmia. A rubéola é responsável por causar catarata, glaucoma, microftalmia e retinite em sal e pimenta. Conclusão: Foram abordadas as principais etiologias infecciosas e seu quadro clínico na CC. O melhor tratamento para CC é cirúrgico associado a acompanhamento clínico, mas a prevenção é a maneira mais eficaz de combater a CC de etiologia infecciosa. O diagnóstico precoce e o tratamento efetivo previnem alterações e sequelas visuais irreversíveis. Nesse contexto, mostram-se importantes as ações de políticas públicas para o melhor desfecho clínico e melhor qualidade de vida.


ABSTRACT Objective: To review the clinical and pathological aspects of CC secondary to infections by syphilis, toxoplasmosis, rubella, cytomegalovirus, herpes simplex. Methods: This is a literature review. Articles from journals indexed to PubMed, COCHRANE, LILACS, EMBASE and SCIELO from 2010 to 2023 were included. Results: A total of 45 articles were found, which, after selection, remained in 9 articles. Some articles were included to enrich the discussion in this topic. The infection caused by syphilis is related to corneal changes. Cytomegalovirus and Toxoplasmosis due to chorioretinitis and/or microphthalmia. Rubella is responsible for causing cataracts, glaucoma, microphthalmia, and salt and pepper retinitis. Conclusion: The main infectious etiologies and their clinical status in CC were addressed. The best treatment for CC is surgery associated with clinical follow-up, but prevention is the most effective way to combat CC of infectious etiology. Early diagnosis and effective treatment prevent irreversible visual changes and sequelae. In this context, public policy actions are important for the best clinical outcome and better quality of life.

Abstract Objective To evaluate the seroprevalence of toxoplasmosis among puerperal women cared for at a tertiary university hospital and the level of understanding of these puerperal women about toxoplasmosis, vertical transmission, and its prophylaxis. Methods For this cross-sectional study, we evaluated 225 patients using presential interviews, prenatal documentation, and electronic medical records. Data were stored using Research Electronic Data Capture (REDCap) software. Prevalence rates were estimated by the presence of reactive IgG antibodies against Toxoplasma gondii. Data analysis was performed using the chi-square test and calculation of the odds ratio (OR). Seroreactivity to T. gondii and exposure variables (age, educational level, and parity) were analyzed using a confidence interval (95%CI) and a significance level of 5% (p < 0.05). Results The seropositivity rate for T. gondii was 40%. There was no association between seroprevalence and age. Primiparity was a protective factor against seropositivity and low education was a risk factor. Conclusion Knowledge of T. gondii infection and its transmission forms was significantly limited, presenting a risk for acute maternal toxoplasmosis and vertical transmission of this protozoan. Increasing the education level regarding the risk of toxoplasmosis during pregnancy could reduce the rates of infection and vertical transmission of this parasite.


Resumo Objetivo Avaliar a soroprevalência de toxoplasmose entre puérperas atendidas em um hospital universitário terciário e o nível de compreensão dessas puérperas sobre toxoplasmose, transmissão vertical e sua profilaxia. Métodos Para esse estudo transversal, foram avaliadas 225 pacientes utilizando entrevistas presenciais, documentação de pré-natal e prontuário eletrônico. Os dados foram armazenados usando o software Research Electronic Data Capture (REDCap). As taxas de prevalência foram estimadas pela presença de anticorpos IgG reativos contra o Toxoplasma gondii. A análise dos dados foi realizada por meio do teste do qui-quadrado e cálculo do odds ratio (OR). A sororreatividade ao T. gondii e as variáveis de exposição (idade, escolaridade e paridade) foram analisadas, utilizando-se intervalo de confiança (IC95%) e nível de significância de 5% (p < 0,05). Resultados A taxa de soropositividade para T. gondii foi de 40%. Não houve associação entre soroprevalência e idade. A primiparidade foi fator de proteção contra a soropositividade e a baixa escolaridade foi fator de risco. Conclusão O conhecimento da infecção por T. gondii e suas formas de transmissão foi significativamente limitado, apresentando risco para toxoplasmose materna aguda e transmissão vertical desse protozoário. Aumentar o nível de escolaridade quanto ao risco de toxoplasmose durante a gravidez pode reduzir as taxas de infecção e transmissão vertical desse parasita.

ABSTRACT

A Neospora caninum e a Toxoplasma gondii são os agentes etiológicos que causam a Neosporose e a Toxoplasmose, respectivamente. Estas duas doenças são consideradas de grande importância econômica e de distribuição mundial, que acometem tanto animais de produção quanto animais domésticos. Apresentam sinais clínicos inespecíficos, sendo a Neosporose frequentemente associada ao abortamento em fêmeas. Ambas enfermidades costumavam ser confundidas, dificultando o diagnóstico. São causadas por protozoários cosmopolitas de ciclos biológicos heteróxenos. O Toxoplasma gondii é responsável por doença clínica em cães e gatos, enquanto o Neospora caninum acomete somente cães. Além disso, não há, até o momento, relatos de Neosporose em humanos, diferente da Toxoplasmose. Ocasionalmente esta pode ocorrer em coiotes, suínos, ovinos, caprinos, equinos, cervídeos e bubalinos. Anticorpos contra Neospora tem sido descrito em raposas, camelos e felídeos. O objetivo da presente revisão, é elucidar a forma de transmissão, sinais clínicos, diagnóstico, tratamento e controle de ambas as doenças, mostrando suas semelhanças, afim de que se possa diagnosticá-las corretamente.(AU)


Neospora caninum and toxoplasma gondii are agents of great economic importance and worldwide distribution that affect production and domestic animals. They present nonspecific clinical signs, and neosporosis is a disease that frequently causes abortion in females, which is considered current, because both used to be confused, making diagnosis difficult. They are protozoan, cosmopolitan, of heterogeneous biological cycles. Toxoplasma gondii is responsible for clinical disease in dogs and cats, while Neospora caninum affects only dogs. Furthermore, there are no reports to date of neosporosis in humans, other than toxoplasmosis. Occasionally it may occur in coyotes, pigs, sheep, goats, horses, deer, and bubaline. Antibodies to Neospora have been described in foxes, camels, and felids. This review aims to elucidate the transmission, clinical signs, diagnosis, treatment, and control of both diseases, showing their similarities, so that they can be correctly diagnosed.(AU)


Neospora caninum y Toxoplasma gondii son los agentes etiológicos que causan Neosporosis y Toxoplasmosis, respectivamente. Estas dos enfermedades se consideran de gran importancia económica y de distribución mundial, afectando tanto al ganado como a los animales domésticos. Presentan signos clínicos inespecíficos y la neosporosis se asocia con frecuencia al aborto en mujeres. Ambas dolencias solían ser erróneas, lo que hacía difícil el diagnóstico. Son causados por protozoos cosmopolitas de ciclos biológicos heterogéneos. Toxoplasma gondii es responsable de enfermedades clínicas en perros y gatos, mientras que Neospora caninum sólo ataca a perros. Además, no se han notificado casos de Neosporosis en humanos hasta el momento, diferente de Toxoplasmosis. Ocasionalmente esto puede ocurrir en coyotes, cerdos, ovejas, cabras, caballos, ciervos y bubalinos. Se han notificado anticuerpos contra la Neospora en zorros, camellos y felinos. El propósito de esta revisión es dilucidar la forma de transmisión, los signos clínicos, el diagnóstico, el tratamiento y el control de ambas enfermedades, mostrando sus similitudes, de manera que puedan ser diagnosticadas correctamente.(AU)

Introduction: Toxoplasmosis is a zoonosis prevalent in one-third of the world's population, adversely affecting maternal-fetal health by causing varying degrees of damage to the fetus. Objective: To assess the seroprevalence of Toxoplasma gondii IgG and IgM antibodies and associated risk factors in first-trimester pregnant women in Cúcuta, Colombia, in 2018. Materials and Methods: Cross-sectional and correlational study in 111 women who voluntarily participated. Specific IgM and IgG antibodies were detected by luminescence immunoassay (LIA). Results: Total seropositivity for IgM and IgG was 19.8% and 35.1%, respectively, 11.7% was for IgM only, and 53.2% corresponded to the overall seronegativity rate for T. gondii. Risk factors (CI = 95%) such as undercooked meat consumption (54.1% of cases, OR = 1.8, p = 0.120), tap water consumption (48.6%, OR = 1.4, p = 0.421), and goat or cow raw milk consumption (39.6%, OR = 0.78, p = 0.553) were identified; in addition, living with cats (23.4%) was identified as a risk factor significantly associated with parasite seropositivity (OR = 2.8, p = 0.025). Discussion and Conclusions: Our findings showed a possible risk of primary infection in more than half of the pregnant population, given the seronegativity against the parasite. A considerable frequency of suspected cases of very recent infection was also found. In addition to being associated with a previously recognized risk factor, this fact suggests the presence of other dietary risks that should be addressed through prevention strategies during prenatal care and the need to strengthen event surveillance.


Introdução: A toxoplasmose é uma zoonose prevalente em um terço da população mundial, que afeta negativamente a saúde materno-fetal, causando diversos graus de danos ao feto. Objetivo: Propôs-se avaliar o estado sorológico IgG e IgM anti - Toxoplasma gondii e fatores de risco relacionados, em gestantes no primeiro trimestre em Cúcuta, Colômbia, no ano de 2018. Material e Métodos: Estudo transversal e correlacional em Participaram 111 mulheres voluntariamente, que foram testadas para IgM e IgG específicos pelo imunoensaio LIA. Resultados: 19,8% e 35,1% do total de soropositividade foram encontrados para IgM e IgG, respectivamente, 11,7% foi apenas para IgM e 53,2% corresponde à frequência de soronegatividade geral para T.gondii; Foram identificados fatores de risco (IC=95%) , como consumo de carne mal cozida (54,1% dos casos, OR= 1,8, p=0,120 ), água da torneira (48,6%, OR=1,4, p=0,421), cru leite de cabra ou vaca (39,6%, OR=0,78, p=0,553 ), além de conviver com gatos (23,4%), sendo este último significativamente associado à soropositividade ao parasita ( OR=2,8 , p =0,025 ). Discussão e conclusões: Nossos achados revelam um possível risco de infecção primária em mais da metade da população gestante dada a sua soronegatividade contra o parasita, mas também uma frequência considerável de casos com suspeita de infecção muito recente, que, além de estar associada a um fator de risco previamente reconhecido , sugere outros aspectos de risco em torno da alimentação que devem ser impactados por meio de estratégias de prevenção durante o pré-natal, sugerindo a necessidade de fortalecer a vigilância em torno do evento.

Una de las principales consecuencias de la infección por Toxoplasma gondii en mujeres embarazadas es la transmisión vertical al feto. Aunque es poco frecuente, la toxoplasmosis congénita puede causar enfermedades neurológicas u oculares graves. La infección primaria por T. gondii durante el embarazo puede tener consecuencias peligrosas, como retinocoroiditis, hidrocefalia, calcificaciones cerebrales, encefalitis, esplenomegalia, pérdida de audición, ceguera y muerte. La atención prenatal debe incluir educación sobre la prevención de la toxoplasmosis. Se trata de un estudio observacional, analítico y transversal. Se evaluaron 209 mujeres gestantes e igual número de recién nacidos; 136 de las mujeres embarazadas resultaron con infección aguda positiva a IgM. De estas 51,20% y 64,71% resultaron primoinfectadas según la determinación de IgA e IgG avidez, respectivamente. 20 de los 35 neonatos provenientes de madres primoinfectadas, adquirieron la infección congénita en el tercer trimestre de la gestación. La conciencia sobre la prevención y el control de la toxoplasmosis es baja entre las poblaciones de alto riesgo. Es necesario fortalecer la educación en salud relacionada con la prevención y el control de la toxoplasmosis en las mujeres en edad reproductiva para prevenir la transmisión vertical a sus productos de gestación y evitar los efectos negativos y hasta mortales de la inefcción por el parásito(AU)


One of the main consequences of Toxoplasma gondii infection in pregnant women is vertical transmission to the fetus. Although rare, congenital toxoplasmosis can cause serious neurological or ocular disease. Primary T. gondii infection during pregnancy can have dangerous consequences, including retinochoroiditis, hydrocephalus, cerebral calcifications, encephalitis, splenomegaly, hearing loss, blindness, and death. Prenatal care should include education on the prevention of toxoplasmosis. This is an observational, analytical and cross-sectional study. 209 pregnant women and the same number of newborns were evaluated; 136 of the pregnant women were acutely infected with IgM. Of these, 51.20% and 64.71% were primary infected according to the determination of IgA and IgG avidity, respectively. 20 of the 35 neonates from mothers with primary infection acquired the congenital infection in the third trimester of pregnancy. Awareness of toxoplasmosis prevention and control is low among high-risk populations. It is necessary to strengthen health education related to the prevention and control of toxoplasmosis in women of reproductive age to prevent vertical transmission to their gestational products and avoid the negative and even fatal effects of infection by the parasite(AU)

ABSTRACT

La toxoplasmosis es una patología parasitaria que es producida por la presencia de un protozoo a nivel intracelular. Tiene varias consecuencias tanto en adultos como en niños, sin embargo, en estos últimos se han evidenciado secuelas como las discapacidades. Objetivo. Realizar una revisión de información de la intervención terapéutica en niños con secuelas de toxoplasmosis. Metodología. Se realizó una revisión bibliográfica en buscadores académicos incluyendo los descriptores: discapacidad, discapacidad física, sensorial e intelectual, intervención psicoterapéutica, intervención terapéutica, toxoplasmosis y toxoplasmosis congénita. Se logró compilar información y se delimitó las secuelas de la toxoplasmosis en dos tipos de discapacidad: intelectual y sensorial. Dentro de esta última, se encontró la prevalencia de las deficiencias a nivel auditivo y visual como consecuencias. Resultados. Se brindó una definición de cada término, así como también la importancia de mantener una perspectiva de derechos e inclusiva al momento de realizar una intervención Psicológica. Conclusiones. A nivel de la Psicología Clínica se identificó que no existe un tratamiento específicamente para las secuelas de toxoplasmosis, sin embargo, el objetivo terapéutico gira en torno a las necesidades individuales de cada sujeto, en este caso, sobre las discapacidades de la población infantil.


Toxoplasmosis is a parasitic pathology that is caused by the presence of a protozoan at the intracellular level. It has several consequences in both adults and children, however, sequelae such as disabilities have been seen in the latter. Objective. To carry out a review of information on therapeutic intervention in children with sequelae of toxoplasmosis. Methodology. A bibliographic review was carried out in academic search engines including the descriptors: disability, physical, sensory and intellectual disability, psychotherapeutic intervention, therapeutic intervention, toxoplasmosis and congenital toxoplasmosis. Information was compiled and the consequences of toxoplasmosis were defined in two types of disability: intellectual and sensory. Within the latter, the prevalence of deficiencies at the auditory and visual level as consequences was found. Results. A definition of each term was provided, as well as the importance of maintaining a rights and inclusive perspective when carrying out a psychological intervention. Conclusions. At the level of Clinical Psychology, it was identified that there is no treatment specifically for the sequelae of toxoplasmosis, however, the therapeutic objective revolves around the individual needs of each subject, in this case, on the disabilities of the child population.


A toxoplasmose é uma patologia parasitária causada pela presença de um protozoário no nível intracelular. Tem várias consequências tanto em adultos como em crianças, no entanto, sequelas como deficiências têm sido observadas nestes últimos. Objetivo. Realizar uma revisão das informações sobre a intervenção terapêutica em crianças com sequelas de toxoplasmose. Metodologia. Foi realizada uma revisão bibliográfica em buscadores acadêmicos incluindo os descritores: deficiência, deficiência física, sensorial e intelectual, intervenção psicoterapêutica, intervenção terapêutica, toxoplasmose e toxoplasmose congênita. As informações foram compiladas e as consequências da toxoplasmose foram definidas em dois tipos de deficiência: intelectual e sensorial. Dentro deste último, encontrou-se a prevalência de deficiências a nível auditivo e visual como consequências. Resultados. Foi fornecida uma definição de cada termo, bem como a importância de manter uma perspectiva de direitos e inclusiva na realização de uma intervenção psicológica. Conclusões. Ao nível da Psicologia Clínica, identificou-se que não existe um tratamento específico para as sequelas da toxoplasmose, contudo, o objetivo terapêutico gira em torno das necessidades individuais de cada sujeito, neste caso, sobre as deficiências da população infantil.

Abstract Objectives: to elaborate and analyze the psychometric properties of a questionnaire to assess among pregnant women's knowledge about toxoplasmosis. Methods: methodological study elaborating an instrument conducted in the north of Minas Gerais in 2019. The steps followed were: 1. Elaboration of the questionnaire items based on medical literature available in indexed databases. 2. Content validation. 3. Apparent validation. 4. Construct validation with hypothesis testing after the application of the questionnaire and comparison of the mean scores using the Mann-Whitney U test. 5. Reliability analysis with internal consistency analysis and test-retest. Results: the final instrument consisted of 26 items. Content validation reached 90% agreement among experts. The hypothesis test found a significant diference among the scores of the groups evaluated (p<0.001). Cronbach's alpha found a value of 0.84 and the test-retest showed an intra-class correlation index of 0.78 (p<0.001). Conclusions: the instrument elaborated proved to be valid and reliable and could be used to assess among pregnant women's knowledge about toxoplasmosis and, therefore, guiding to the educational and preventive measures.


Resumo Objetivos: elaborar e analisar as propriedades psicométricasde um questionáriopara avaliação do conhecimento sobre toxoplasmose entre gestantes. Métodos: estudo metodológico de elaboração de instrumento, realizado no norte de Minas Gerais, em 2019. O estudo seguiu os seguintes passos: 1. Elaboração dos itens a do questionário, com base na literatura médica disponível em bases indexadas. 2. Validação de conteúdo. 3. Validação aparente. 4. Validação de constructo, com teste de hipóteses, após aplicação do questionário e comparação de médias dos escores por meio do teste U de Mann-Whitney. 5. Análise de confabilidade, com análise da consistência interna e teste-reteste. Resultados: o instrumento final foi composto por 26 itens. Na validação de conteúdo alcançou 90% de concordância entre os experts. O teste de hipótese registrou diferença significativa entre os escores dos grupos avaliados (p<0,001). O alfa de Cronbach apresentou valor de 0,84 e o teste-reteste apresentou índice de correlação intra-classe de 0,78 (p<0,001). Conclusão: o instrumento elaborado apresentou-se válido e confável e poderá ser utilizado para avaliação de conhecimento sobre toxoplasmose entre gestantes, norteando medidas educativas e preventivas.

O vírus da imunodeficiência humana é o agente etiológico da AIDS, doença crônica que destrói o sistema imunológico e é caracterizada pela baixa contagem de células TCD4, alta contagem de partículas virais no sangue e manifestações clínicas da doença. O diagnóstico se dá com o aparecimento de infecções oportunistas, que levam a contagem de TCD4 a níveis menores que 200 céls/mm³. Os exames laboratoriais para o diagnóstico do HIV foram os principais avanços para o início do tratamento, reduzindo a transmissão. Detecção de anticorpos, detecção de antígenos e amplificação do genoma do vírus são alguns dos exames laboratoriais utilizados para diagnóstico. Os dois principais biomarcadores são os exames de contagem de células TCD4, que verifica o sistema imune, e a quantificação de carga viral, que informa a quantidade de partículas virais, mostrando a progressão da infecção. Quanto maior a carga viral, maior o dano ao sistema imune. Uma carga viral indetectável é inferior a 50 cópias/mL, mas valores menores ou iguais a 200 cópias/mL também impedem a transmissão. Uma declaração de consenso afirma que Indetectável é igual a Intransmissível. Portanto, quando indetectável, a transmissão inexiste. O presente estudo relata e discute o caso clínico de uma paciente diagnosticada com HIV/AIDS aos 28 anos, que sobreviveu, apesar do diagnóstico tardio, e sob presença de doença oportunista com um grave grau de diminuição de células TCD4 (22 cél/mm³). Por meio do diagnóstico, introdução e adesão correta da terapia antirretroviral e monitorização de exames laboratoriais, conseguiu evitar a morte e ter uma vida semelhante à de um HIV negativo. Ultrapassou a expectativa de vida que na descoberta era de 10 anos, com uma qualidade de vida considerável, não sendo transmissora do vírus, diminuindo assim o estigma e preconceito. O biomédico é peça fundamental nesse contexto, considerando que deve fornecer informações precisas e fidedignas, tão necessárias ao acompanhamento de pessoas vivendo com HIV, para que autoridades e profissionais de saúde adotem medidas adequadas, tanto na prevenção, quanto no diagnóstico e monitoramento da doença.


The human immunodeficiency virus is the etiological agent of AIDS, a chronic disease that destroys the immune system and is characterized by low TCD4 cell count, high viral particle count in blood and clinical manifestations of the disease. The diagnosis is due to the appearance of opportunistic infections, which lead to TCD4 counts below 200 cells / mm³. Laboratory tests for the diagnosis of HIV were the main advances in starting treatment, reducing transmission. Antibody detection, antigen detection and virus genome amplification are some of the laboratory tests used for diagnosis. The two main biomarkers are the TCD4 cell count tests, which checks the immune system, and viral load quantification, which reports the number of viral particles, showing the progression of infection. The higher the viral load, the greater the damage to the immune system. An undetectable viral load is less than 50 copies / mL, but values less than or equal to 200 copies / mL also prevent transmission. A consensus statement states that Undetectable equals Non-Transmissible. Therefore, when undetectable, transmission does not exist. The present study reports and discusses the clinical case of a patient diagnosed with HIV / AIDS at age 28, who survived despite late diagnosis and under the presence of opportunistic disease with a severe degree of TCD4 cell reduction (22 cells / mm³). Through the diagnosis, introduction and correct adherence of antiretroviral therapy and monitoring of laboratory tests, she was able to avoid death and have a life similar to that of an HIV negative. Exceeded the life expectancy that in the discovery was 10 years, with a considerable quality of life, not transmitting the virus, thus reducing the stigma and prejudice. The biomedical is a key player in this context, considering that he must provide accurate and reliable information, which is so necessary for the monitoring of people living with HIV, so that authorities and health professionals adopt appropriate measures, both in prevention, diagnosis and monitoring of the disease.

INTRODUCCIÓN: La toxoplasmosis es una enfermedad zoonótica cuyo contagio se produce por vía oral por ingestión de quistes al consumir carne contaminada o al tener contacto con heces de felinos contaminadas con ooquistes. Con menor frecuencia, se adquiere por transmisión vertical o por trasplante de órganos infectados. En Chile, existen estudios serológicos en personas sanas e inmunodeprimidas, pero no en grupos de riesgo ocupacional, como son los estudiantes de Medicina Veterinaria. OBJETIVO: Determinar la seroprevalencia para T. gondii en estudiantes de Medicina Veterinaria de una universidad ubicada en el centro-sur del país, y ver su asociación con variable intrínsecas. MÉTODOS. Se tomaron muestras de sangre a 74 estudiantes de una universidad del centro-sur de Chile. Se determinó la presencia de anticuerpos específicos clase IgG contra Toxoplasma gondii mediante la técnica de inmunoanálisis por quimioluminiscencia en fase solida secuencial. RESULTADOS. Del total de alumnos, 16 fueron seropositivos lo que equivale a 21,6%, el mayor número de estudiantes seropositivos fue de sexo femenino (75%), se encontraban en el rango etario entre 24 y 26 años de edad (43,7%), todos consumían carne y verduras (100%) y realizaban la recolección de heces de gatos sin protección 50 (68,7%). CONCLUSIONES. Los resultados obtenidos demuestran la presencia de anticuerpos contra T. gondii en alumnos de Medicina Veterinaria. Según el conocimiento de los autores, es el primer estudio serológico para toxoplasmosis realizado en estudiantes universitarios de Medicina Veterinaria en Chile.


BACKGROUND: Toxoplasmosis is a disease that results from infection with the Toxoplasma gondii parasite, one of the world's most common parasites. The zoonotic infection usually occurs by eating undercooked contaminated meat or by contact with contaminated cat feces. A few studies of toxoplasmosis in healthy and immuno-compromised persons were done in Chile, but investigations related to occupational risk groups, such as Veterinary Medicine students were not studied. AIM: Determine the seroprevalence for T. gondii in Veterinary Medicine students at a university located in the center-south of the country, and evaluated to association with intrinsic variables. METHODS: Peripheral blood samples were taken from 74 veterinary students from a university in south-central Chile. Immunoassay with sequential chemiluminescent was used as diagnostic technique. RESULTS. Of the total number of students, 16 were seropositive, which is equivalent to 21.6%, the largest number of seropositive students was female (75%), they were in the age range between 24 and 26 years of age (43.7%), all consumed meat and vegetables (100%) and collected pet cat feces without protection 50 (68,7%). CONCLUSIONS: The results obtained show the presence of antibodies against T. gondii in Veterinary Medicine students. To the authors' knowledge, it is the first serological study for toxoplasmosis performed in university students of Veterinary Medicine in Chile.

ABSTRACT

Resumen La toxoplasmosis es una de las parasitosis con mayor prevalencia en el mundo. Se asocia a morbimortalidad perinatal y en población inmunocomprometida. Actualmente se cuenta con varias pruebas diagnósticas, tanto serológicas como moleculares, que pueden ayudar a confirmar el diagnóstico y diferenciar una infección reciente de una pasada. Proponemos algunas recomendaciones para el diagnóstico y tratamiento de la toxoplasmosis en distintos escenarios clínicos basados en la evidencia actualmente disponible.


Abstract Toxoplasmosis is one of the most prevalent parasitosis in the world. It is associated with perinatal morbidity and in immunocompromised population. Currently, there are several diagnostic tests, both serological and molecular, that can help confirm the diagnosis and differentiate a recent infection of a pass. We propose some recommendations for the diagnosis and treatment of toxoplasmosis in different clinical scenarios based on the currently available evidence.

This study aimed to determine the prevalence of Toxoplasma gondii (T. gondii) infection in pregnant women in Santa Catarina, as well as to analyze the knowledge about zoonosis in the study population. Methods: Therefore, 109 volunteers evaluated in Basic Health Units in the city of Blumenau and Brusque were evaluated. The data were collected through questionnaires and analysis of the pregnant woman's handbooks with medical records. Results: In this study, the prevalence of IgG anti-T. gondii was 53.7%, with positivity for IgM anti-T. gondii was 1.0%. Seronegativity for toxoplasmosis was verified in 45.3% of the participants. In addition, the data demonstrated that the majority of pregnant women have not any knowledge of the disease. Conclusion: The results of obtained demonstrated that health actions promotion is crucial to clarify about T. gondii infection, in order to prevent this disease during pregnancy.


O presente trabalho teve por objetivo determinar a prevalência de infecção pelo Toxoplasma gondii (T. gondii) entre gestantes de Santa Catarina, bem como analisar o conhecimento sobre a zoonose na população em estudo. Métodos: Para tanto, foram avaliadas 109 voluntárias atendidas em Unidades Básicas de Saúde dos municípios de Blumenau e Brusque. Os dados foram coletados por meio de questionários e análise de cadernetas da gestante. Resultados: Neste estudo, a prevalência global de anti-T. gondii IgG foi de 53,7%, sendo que a positividade para anti-T. gondii IgM foi de 1,0%. A soronegatividade para toxoplasmose foi verificada em 45,3% das participantes. Além disso, os dados revelaram que a maioria das gestantes não tem conhecimento acerca da doença. Conclusão: Os resultados deste estudo demostraram a necessidade da promoção de ações de saúde voltadas ao esclarecimento sobre o T. gondii visando prevenir a infecção durante a gestação.

Introducción. Toxoplasma gondii es un parásito con gran potencial zoonótico que puede infectar un amplio rango de huéspedes de sangre caliente, incluidos los animales del sector pecuario, lo que causa pérdidas a la industria. En el humano, es patógeno en personas inmunosuprimidas y afecta el desarrollo del feto en infecciones congénitas. Además, se asocia con diversos trastornos del comportamiento en personas sanas. El humano puede adquirir T. gondii al consumir carnes contaminadas mal cocidas. Objetivo. Determinar la positividad de T. gondii en carnes de consumo humano (res, pollo y cerdo) en Ibagué, Colombia. Materiales y métodos. Se utilizó la PCR convencional anidada y la secuencia del gen B1 de T. gondii como blanco de amplificación. Se tomaron 186 muestras de carne comercializada en la zona urbana de Ibagué (62 de res, 62 de pollo y 62 de cerdo) y se obtuvo el porcentaje de positividad en cada tipo de carne evaluada. Resultados. Se encontró un porcentaje de positividad de 18,8 % en las muestras, siendo la carne de cerdo la del mayor porcentaje (22,5 %; 14/62), seguida por las muestras de carne de res (19,3 %; 12/62) y de pollo (14,5 %; 9/62). Los mejores productos amplificados fueron secuenciados en Macrogen, y alineados con las secuencias del gen B1 depositadas en el GenBank, con lo que se confirmó su identidad. Conclusiones. Este es el primer estudio sobre prevalencia de T. gondii en carnes para consumo humano en Ibagué y el departamento del Tolima. Se demostró que los tres tipos de carne representan un riesgo para la infección en humanos a nivel local.


Introduction: Toxoplasma gondii is a parasite with great zoonotic potential. It can infect a broad range of warm-blooded hosts (including livestock) and causes significant losses in the industry. In humans, it has been described as a pathogen in immunosuppressed people, it affects the fetus development in congenital infections, and is associated with various behavioral disorders in healthy people. Humans can acquire T. gondii by consuming undercooked, contaminated meat. Objective: To determine T. gondii positivity (currently unknown) in meat for human consumption (i.e., beef, chicken, and pork) in the city of Ibague, Colombia. Materials and methods: We used conventional nested PCR and the T. gondii B1 gene sequence as amplification target. We collected samples of meat (N=186) sold in the urban area of Ibagué (62 beef, 62 chicken, and 62 pork samples) and determined the T. gondii positivity percentage for each type of meat. Results: The study found an average of 18.8% positivity for all meat samples, pork having the highest percentage (22.5%; 14/62), followed by beef (19.3%; 12/62) and chicken (14.5%; 9/62). The best-amplified products were sequenced by macrogen and aligned with the B1 gene sequences in GenBank, thereby confirming their identity. Conclusions: This is the first study of T. gondii prevalence in meat for human consumption carried out in the city of Ibagué and the department of Tolima. All three types of meat sampled represent a risk for local human infection.

@#A 36-year-old-male was admitted complaining of headache, right sided weakness and numbness of upper and lower extremity, and multiple cranial nerve deficits. Cranial magnetic resonance imaging revealed an abscess in the pontomesencephalic junction. Patient was then diagnosed to have Human Immunodeficiency Virus with a CD 4 count of 32 cells/ uL, his CSF assay was positive for Toxoplasmosis IgG and was managed as a case of probable brainstem toxoplasmosis. Patient was treated with Co-Trimoxazole 800/160mg 2 tablets twice a day. Upon discharge the patient clinically improved and was tolerating oral feeding. A repeat cranial magnetic resonance imaging after 6 weeks of antibiotic treatment revealed a decrease of size in the previous lesion. To our knowledge, there are no reported cases in the Philippines that shows the documentation of CNS toxoplasmosis in the brainstem. In this paper, a case of CNS toxoplasmosis in the pons of a newly diagnosed HIV patient is presented and how its course led to a good outcome.

La toxoplasmosis es una infección parasitaria con distribución mundial, cuyo huésped definitivo son los felinos. Se presenta con un cuadro clínico sintomático difuso solo en el 20 % de los pacientes inmunocompetentes. Además del malestar general, fiebre y adinamia, el hallazgo más importante lo constituye la presencia de adenopatías a nivel cervical. Describimos un caso poco común de un paciente masculino con masa parotídea izquierda de crecimiento rápido, en quien se practicó parotidectomía, a fin de descartar la enfermedad linfoproliferativa. Se comprobó compromiso por Toxoplasma gondii luego de múltiples análisis.


Toxoplasmosis is a parasitic infection with worldwide distribution, whose definitive host is felines. It is only symptomatic in less than 20% of immunocompetent patients. Symptoms may vary from general malaise, fever, weakness and the presence of cervical lymphadenopathy. A description of an uncommon presentation of this disease in a male patient with progressively growing left-sided parotid mass, in whom a parotidectomy was performed to rule out lymphoproliferative disease. Infection by Toxoplasma gondii was diagnosed after a series of exams

ABSTRACT

Toxoplasma gondii is a worldwide parasite that can infect almost all kinds of mammals and cause fatal toxoplasmosis in immunocompromised patients. Apoptosis is one of the principal strategies of host cells to clear pathogens and maintain organismal homeostasis, but the mechanism of cell apoptosis induced by T. gondii remains obscure. To explore the apoptosis influenced by T. gondii, Vero cells infected or uninfected with the parasite were subjected to apoptosis detection and subsequent dual RNA sequencing (RNA-seq). Using high-throughput Illumina sequencing and bioinformatics analysis, we found that pro-apoptosis genes such as DNA damage-inducible transcript 3 (DDIT3), growth arrest and DNA damage-inducible α (GADD45A), caspase-3 (CASP3), and high-temperature requirement protease A2 (HtrA2) were upregulated, and anti-apoptosis genes such as poly(adenosine diphosphate (ADP)-ribose) polymerase family member 3 (PARP3), B-cell lymphoma 2 (Bcl-2), and baculoviral inhibitor of apoptosis protein (IAP) repeat containing 5 (BIRC5) were downregulated. Besides, tumor necrosis factor (TNF) receptor-associated factor 1 (TRAF1), TRAF2, TNF receptor superfamily member 10b (TNFRSF10b), disabled homolog 2 (DAB2)‍-interacting protein (DAB2IP), and inositol 1,4,5-trisphosphate receptor type 3 (ITPR3) were enriched in the upstream of TNF, TNF-related apoptosis-inducing ligand (TRAIL), and endoplasmic reticulum (ER) stress pathways, and TRAIL-receptor 2 (TRAIL-R2) was regarded as an important membrane receptor influenced by T. gondii that had not been previously considered. In conclusion, the T. gondii RH strain could promote and mediate apoptosis through multiple pathways mentioned above in Vero cells. Our findings improve the understanding of the T. gondii infection process through providing new insights into the related cellular apoptosis mechanisms.

Toxoplasmosis is caused by infection with the protozoan parasite Toxoplasma gondii, that has the capacity to infect all warm-blooded animals worldwide. The purpose of this investigation was to determine the distribution of genotypes and alleles in miscarriages woman as a result of Toxoplasma gondii infection associated with interleukin-1β and interleukin-6 polymorphisms. A total of 125 miscarriage women suspected of toxoplasma infection and 50 healthy pregnant without previous miscarriage as control were enrolled in this study. The cases were screened for anti-toxoplasma IgM and IgG by ELISA test. Among the 125 miscarriage women, only 50 were positive to anti-Toxoplasma gondii IgG and IgM antibodies. The present study focused on assay the genotypes at IL-6 -174 G/C and IL-1β +3954 G>A to establish the associations between genetic polymorphisms and infection with Toxoplasma gondii. Results showed that the altered IL-1β GA, AA genotypes were high significant elevated in miscarriage women with toxoplasmosis (P=0.03), OR = 10 and 95 percent confidence intervals (1.32-81.48); (P=0.0007), OR = 0.07 and 95 percent confidence interval (0.01-0.32). The genotype GC at IL-6 (G/C) appears to be highly correlated with infection (P=0.01); OR = 3.18 and 95 percent confidence interval, (1.22- 8.30). In terms of allelic heterogeneity, C alleles were significantly more common in infected than uninfected cases for IL-6, while A allele is common in IL-1β single nucleotide polymorphisms (P =0.050). Furthermore, this study demonstrates that there is a strong and highly significant association between two forms of single nucleotide polymorphisms and the increased risk for toxoplasmosis. Genotypes of these polymorphism should be considered when evaluating genetic effects on toxoplasmosis incidence. However, to improve the prediction of this disease predisposition, a further study based on a larger cohort of patients is warranted(AU)


La toxoplasmosis es causada por la infección con el parásito protozoario Toxoplasma gondii, que tiene la capacidad de infectar a todos los animales de sangre caliente en todo el mundo. El propósito de esta investigación fue determinar la distribución de genotipos y alelos en mujeres con abortos espontáneos como resultado de la infección por Toxoplasma gondii asociada con polimorfismos de interleucina 1β e interleucina 6. Se inscribieron en este estudio un total de 125 mujeres con aborto espontáneo sospechosas de infección por toxoplasma y 50 embarazadas sanas, sin aborto espontáneo previo, como control. Los casos se examinaron para detectar IgM e IgG anti-toxoplasma mediante la prueba ELISA. Entre las 125 mujeres que sufrieron un aborto espontáneo, solo 50 fueron positivas a anticuerpos IgG e IgM anti-Toxoplasma gondii. El presente estudio se centró en analizar los genotipos de IL-6-174 G/C e IL-1β +3954 G>A para establecer las asociaciones entre polimorfismos genéticos e infección por Toxoplasma gondii. Los resultados mostraron que los genotipos alterados de IL-1β GA, AA fueron significativamente elevados en mujeres con aborto espontáneo con toxoplasmosis (P = 0,03), OR = 10 e intervalos de confianza del 95 por ciento (1,32-81,48); (P = 0,0007), OR = 0,07 e intervalo de confianza del 95 por ciento (0,01-0,32). El genotipo GC de IL-6 (G/C) parece estar altamente correlacionado con la infección (P = 0.01); OR = 3,18 e intervalo de confianza del 95%, (1,22- 8,30). Se deben considerar los genotipos de estos polimorfismos al evaluar los efectos genéticos sobre la incidencia de la toxoplasmosis. Sin embargo, para mejorar la predicción de esta predisposición a la enfermedad, se justifica un estudio adicional basado en una cohorte más grande de pacientes(AU)

Introduction: There is a need for studies to know the real situation and outline measures to guarantee a reduction in the rates of pregnant women diagnosed with HIV, Syphilis and Toxoplasmosis. Objective: To determine the prevalence of Syphilis, HIV and toxoplasmosis in puerperal women assisted at the largest public maternity hospital in Campos dos Goytacazes in 2016. Methods: Cross-sectional study, using secondary data from the puerperal women assisted at the largest maternity hospital in the northern region of the State of Rio de Janeiro, in the year 2016. Results: There were 970 puerperal women, with a prevalence of HIV, Syphilis and Toxoplasmosis in pregnancy of 1.6, 2.7 and 2%, respectively. Most pregnant women were diagnosed at delivery due to low serological coverage during pregnancy. Conclusion: The high prevalence of Syphilis, HIV and Toxoplasmosis in pregnancy requires efficient prenatal care for its identification and approach.


Introdução: São necessários estudos para conhecer a real situação e delinear medidas que garantam a redução dos índices de gestantes com diagnóstico de vírus da imunodeficiência humana, sífilis e toxoplasmose. Objetivo: Determinar a prevalência de sífilis, vírus da imunodeficiência humana e toxoplasmose em puérperas atendidas na maior maternidade pública de Campos dos Goytacazes (RJ) no ano de 2016. Métodos: Estudo transversal, utilizando dados secundários das puérperas atendidas na maior maternidade do Norte Fluminense no ano de 2016. Resultados: Foram 970 puérperas, com prevalência de vírus da imunodeficiência humana, sífilis e toxoplasmose na gestação de 1,6, 2,7 e 2%, respectivamente. A maioria das gestantes foi diagnosticada no momento do parto em razão da baixa cobertura sorológica durante a gestação. Conclusão: A alta prevalência de sífilis, vírus da imunodeficiência humana e toxoplasmose na gravidez requer um pré-natal eficiente para sua identificação e abordagem.

Toxoplasma gondii is a protozoan parasite with worldwide distribution. Dogs are considered sentinel animals, useful for the detection of environmental infections and outbreaks of disease through serological surveys. Considering this, we aimed to assess the seroprevalence of toxoplasmosis in dogs from rural areas of the hinterland of the state of Sergipe, Northeastern Brazil. A total of 77 blood samples were collected from dogs and analyzed by the Indirect Hemagglutination test for the detection of anti-T. gondii antibodies Samples were considered positive when titers greater than or equal to 1:16 were observed. In addition, an epidemiological questionnaire designed to identify characteristics of the animal was performed to owners regarding sex, breed, age, sterilization, access to areas outside the house, environmental hygiene, contact with other animals and dog food. A total of 49.3% of dogs (n = 38) were positive for anti-T. gondii antibodies, most with high titers (44.7%). A higher relative frequency of seropositivity was observed in male dogs, not sterilized, mixed breed, which were fed exclusively on homemade food and had access to the street and contact with other animals. Taken together, our data reinforce the importance of developing health education actions within the scope of public health aimed at preventing this zoonosis.


Toxoplasma gondii é um protozoário com distribuição mundial. Os cães são considerados animais sentinela, úteis para a detecção de infecções ambientais e surtos da doença por meio de levantamentos sorológicos. Portanto, o presente estudo teve como objetivo determinar a soroprevalência da toxoplasmose em cães da zona rural da região rural do Alto Sertão Sergipano, Brasil. Um total de 77 amostras de sangue de cães foram coletadas no município de Nossa Senhora da Glória, Sergipe e submetidas ao teste de Hemaglutinação Indireta para detecção de anticorpos anti-T. gondii, considerando títulos maiores ou iguais a 1:16 como positivos. Além disso, foi aplicado um questionário epidemiológico aos proprietários solicitando informações sobre sexo, raça, idade, esterilização, acesso às ruas, limpeza do ambiente, contato com outros animais e ração, além de dados do proprietário, como renda familiar e nível de educação. 49,3% dos cães (38/77) apresentaram anticorpos anti-T. gondii, a maioria com títulos elevados (44,7%). Foi observado maior frequência relativa de soropositividade em cães macho, não esterilizados, sem raça definida, que se alimentam exclusivamente de comida caseira e tem acesso a rua e contato com outros animais. Os dados obtidos reforçam a importância do desenvolvimento de ações de educação em saúde no âmbito da saúde pública voltadas à prevenção dessa zoonose.

La toxoplasmosis es causada por un protozoo intracelular, Toxoplasma gondii, que tiene una amplia distribución geográfica. La forma congénita resulta en una forma gestacional que puede presentar una parasitemia temporal que infectará al feto. Es adquirido por consumo de carne cruda o poco cocida, alimentos o agua contaminada e infección congénita a través de la placenta. El objetivo de este estudio fue determinar la prevalencia de infección por toxoplasmosis y sus posibles factores de riesgo asociados con mujeres embarazadas que asistieron al servicio de control prenatal en el Hospital Gineco Obstétrico "Isidro Ayora", Ecuador. Las muestras de sangre se analizaron para detectar la presencia de anticuerpos IgG e IgM contra T. gondii utilizando una prueba de hemoaglutinación. Se probó la significación de los factores de riesgo mediante análisis bivariado. Se consideró un valor de p <0,05 como estadísticamente significativo. La prevalencia de anticuerpos IgG e IgM fue de 16,32%. Se observó una asociación significativa entre la seroprevalencia y antecedentes de aborto (p=0,00804), contacto con gatos domésticos (p<0,0001) y hábitos higiénicos incorrectos (p<0.0001). Los hallazgos demostraron que el contacto con gatos, antecedentes de abortos y hábitos higiénicos incorrectos se identificaron como factores de riesgo de infección por T. gondii(AU)


Toxoplasmosis is caused by an intracellular protozoan, Toxoplasma gondii, which has a wide geographical distribution. The congenital form results in a gestational form that can present a temporary parasiteamia that will infect the fetus. It is acquired by consumption of raw or undercooked meat, food or water contaminated and congenital infection through the placenta. Tthe objective of this study was to determine the prevalence of toxoplasmosis infection and its possible risk factors associated with pregnant women who attended the prenatal control service at the Gyneco-Obstetric Hospital "Isidro Ayora", Ecuador. Blood samples were tested for presence of IgG and IgM antibodies against T. gondii using a hemagglutination test. The risk factors were tested for significance using bivariate analysis. P-value <0.05 was considered statistically significant. The prevalence of IgG and IgM antibodies was 16.32%. A significant association was observed between seroprevalence and history of abortion (p=0,00804), contact with domestic cats (p<0,0001) and incorrect hygiene habits (p<0,0001). The findings demonstrated that contact with cats, a history of miscarriages, and poor hygiene habits were identified as risk factors for T. gondii infection(AU)
